Why
we do it
The National Service Framework for CHD has set a series of standards.
It is intended that this approach will contribute to the national
targets for CHD and Stroke, set out in Our Healthier Nation.
The need to tackle risk factors for CHD has also been highlighted
in the Local Modernisation Reviews. Within the review specific targets
have been designed to work towards improving inequalities in health.
The actions include: the development of a smoking cessation service,
the reduction in the levels of obesity and increased opportunities
for healthy eating, the development of regular physical activity
opportunities.
How we do it
Supporting the work of the PCTs by sharing good and advising on
appropriate health promotion interventions:
Exercise for Health
This award winning GP referral scheme targets clients who have two
or more risk factors for coronary heart disease. It provides, supervised
exercise within the local authority leisure centres.
Walk for Health - Liverpool
This successful scheme is now available across the three PCT areas.

Cycle for Health - Lverpool
This scheme will initially be piloted in North Liverpool PCT.
It aims make use of green space and the loopline system. It will
provide
guided cycle routes, training and support to community organisations
who wish to set up a cycle group. For further information

Expert patient
A nationally recognised course seeking to provide clients,
with skills to enable them to self manage their health condition.
The course consists of 6 weekly sessions, each lasting two
and half
hours.
Consultation
Providing advice and support to local community
health forums, to develop
their own priorities.
Campaigns
Identifying and securing resources which will assist community and
primary health care workers to carry out their health promotion
events.
